2024-25 Health Care Plan

Life-Threatening Health Care Plans (HCP)

If your student has a life-threatening health condition, you must have a current 2024-2025 HCP with necessary doctor’s orders and medication on file in the Health Office before the first day of school. These items must be dropped off to the office no later than August 15 in order to allow the nurse ample time to process this important information. Per Washington State Law and Issaquah School District regulations, your student will not be able to attend school until these items are turned in.

The State Board of Health has approved changes to Chapter 246-105 of the Washington Administrative Code (WAC) concerning school and child care immunization policy.  These rules apply to all public and private schools and licensed child care centers in Washington State.

The update to Chapter 246-105 WAC has three major focuses:

  • It clarifies conditional status in regards to school and child care immunization requirements and implementation.
  • It changes the Tdap immunization requirement to 7th through 12th grades.
